EMPLOYEE CENSUS

In order for us to obtain an accurate quote, please complete the Employee Census and be as detailed as possible. We will need the business name, zip code and all employee’s dates of birth along with their family members and date of birth.

A census does not include health status, race, religion, sexual orientation (even if applying for domestic partner benefits), Social Security number, or U.S. citizenship/immigration status.

If the employee or family member isn’t requiring coverage, you can indicate on the census that you’d like to waive coverage for those individuals.
